It happened to me during my NS days live firing of 106mm recoiless gun. The round requires the gunner to cradle it like a baby to load into the gun.

This particular trainee happened to carry the round but it slipped from his arms. This round is fired by hitting the percussion cap in the gun and it landed on the rocky ground hitting the cap. As instructors, we were standing beside the gun and everyone just froze, stared at the round as it landed on its side after hitting the cap.

Good thing it did not explode.
